NATO Orders - Passports/Visas - Base Pass - Dependent ID Badges - E-mail Set Up
You will need NATO orders to board the plane.
These orders can be done by either your unit CSS or the MPF.
Procedures vary by organization so make sure you understand who is cutting the orders and where you get them.
The dates on the orders should encompass your entire anticipated tour length at Lajes.
Orders must be original signatures in BLUE ink and have an official stamp.
Military members do not require a passport for travel to Lajes Field.
All family members who are U.S. citizens and will accompany you MUST have a valid U.S. No-Fee Passport. This is a passport issued for 5 years.Employees are responsible for ensuring that their passport and those of their dependents are current and valid. An individual with an expired passport will not be allowed entrance into Portugal. Once you have arrived at Lajes Field, Azores you must have a permanent visa stamped on your passport and your dependents passports. You have 90 days to obtain a visa. Once you arrive we will inform you of where to go to obtain a visa. Non-U.S. Citizens must possess a valid passport from the country of their citizenship. Tourist passports are blue passports which look the same as the No-Fee Passport, but are issued for 10 years. They should not be used for PCS travel. Ensure your Outbound Assignments Clerk processes No-Fee passports for your family members. Start this process as soon as possible; obtaining your family’s passport can take some time.
Base pass is required for all dependents (over 10) to be displayed while on base. Complete the form prior to arrival. Download Base Pass Request.

It's highly encouraged for military members to apply for a tourist passport if you have intentions of visiting Europe. MPS can help you obtain a passport.
All military dependents (spouses/children, etc) over the age of 10 MUST get this new ID card in addition to their military ID Card. It will be worn during our duty hours 0800-1700, M-F. They will go to the glass Portuguese bldg next to the main gate (towards Praia) to get the IDs. Their business Hrs are 0800-1200 & 1300-1545 M-F. Mid-morning/mid-afternoon are slow times for them. AB4 will provide the little metal alligator clip that loops on the card. Dependents do NOT need to have the sponsor with them to drop off application, take photos, or pick up badge, however, if sponsor is NOT accompanying the dependents then the SQ CC should sign & print their name & position on the bottom of AB4 Application (sponsor block).
STEP 1. PCSing member completes forms required to expedite creation of new user account:
AF Form 4394, Air Force User Agreement Statement - Notice and Consent Provision
USAFE Form 115, Network User Agreement and Account Request
Information Awareness Training Certificate (NOTE: Training must have been accomplished within 1 year)
STEP 2. PCSing member forwards forms to gaining supervisor for
verification. IMPORTANT NOTE: Supervisors must ensure Section III, Block 10(a)(b)(c) is populated.
STEP3. Supervisor forwards documents to Squadron Information Awareness Officer (IAO) for processing. NOTE: If member is PCSing from another USAFE base, there will be a delay in creating new account.
